Community Care Advocate: Social Resources Navigator

Spring Health seeks a Community Care Advocate to connect employees with community and company resources addressing social determinants of health and employee needs. This onsite role requires schedule Mon-Fri in Chicago ZIP View phone number on click.appcast.io, with referrals, crisis support, and navigation of mental health benefits. You’ll refer with internal care teams, document outcomes, and promote the service to members while using tech-enabled tools.
