Location Greenwood, 8741 South Greenwood, Chicago, IL 60619, USA Overview Family Support Program (FSP), formerly known as the Individual Care Grant program, provides access to intensive mental health services and supports to youth with a severe emotional disturbance. The goal of the FSP is to support eligible youth and their families by strengthening family stability, improving clinical outcomes, and promoting community-based services. The Specialized Family Support Program (SFSP) is a 90‑day program of crisis stabilization, community mental health, and assessment services. It is designed to identify the behavioral health needs of youth at risk of custody relinquishment (i.e., psychiatric hospital lockout) and to link those youth to the most appropriate clinical services. Basic Job Function Under the supervision of a Licensed Practitioner of the Healing Arts (LPHA), the FSP/SFSP Coordinator may provide clinical assessment, treatment planning, crisis intervention, coordination with the Illinois Department of Healthcare and Family Services (HFS) and residential entities, collaboration with hospital staff, case management, community support, therapy/counseling, and administer direct clinical interventions to children, adolescents, and their families experiencing acute and chronic emotional/behavioral disorders. The Coordinator will provide these services primarily within the home or community. Reporting Relationship Reports To: Clinical Supervisor and Behavior Health Manager Responsibilities Maintain knowledge of FSP criteria and requirements, communicate with potential FSP candidates, and enroll them into the program. Once approved, maintain their enrollment and link the recipient to community‑based and/or residential services. Maintain weekly verbal communication with client and family; complete and submit continued enrollments, prior authorizations, and applications as needed in accordance with HFS protocol. Maintain knowledge of SFSP criteria and requirements; enroll youth into the program, gather all required documentation, and coordinate with hospital staff until discharge into parents’ care. Provide support and direct services to the family for 90 days to maintain stabilization at home. Complete required HFS reports. Perform Integrated Assessments and Treatment Plans (IATP) for children and adolescents referred for FSP/SFSP. Develop and provide a diagnostic assessment consistent with all Axis of the current Diagnostic and Statistical Manual and International Classification of Diseases, or their successors. Ensure all services provided comply with informed consent, client rights, and confidentiality standards. Implement direct services included in the client’s treatment plan: clinical therapeutic rehabilitative services, case management, community support, crisis intervention, and mental health stabilization. Consult with other professional service providers in accordance with the current administrative code. Prepare and oversee case presentation for clinical review at specified points of care: initial assessments, treatment planning review, treatment staffing reviews, and case closure. Link clients with appropriate treatment, educational, social, recreational, and medical resources during and prior to completion of service. Maintain current knowledge of services for children, adolescents, adults, and families; participate in agency in‑service trainings and advanced skill‑trainings. Adhere to current rules and regulations pertaining to services delivered by the Illinois Department of Human Services and related agencies. Collaborate at an advanced level with colleagues, consultants, intra‑ and inter‑agency service coordination, and community‑planning and advocacy on behalf of clients served. Document service provisions consistent with the treatment plan and ensure all identified services are provided in accordance with the agency or program, and current administrative codes for community mental health treatment services. Maintain client confidentiality and adhere to HIPAA guidelines. Perform other duties commensurate with responsibilities outlined herein. Maintain current and accurate documentation of client services (billable/non‑billable), client scheduling, consultation, and other events related to the electronic health record. Specifications This position requires a minimum of a master’s degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, family therapy, or a related field as defined by the governing mental health authority. Must have successfully completed a practicum and/or internship which includes a minimum of 1,000 hours, or one year of clinical experience under a Licensed Qualified Mental Health Professional, including QMHP physician’s, licensed clinical psychologists, or registered nurses licensed under the Illinois Nursing Act of 1987 with at least one year of clinical experience in a mental health setting. The position requires a personal auto, valid driver’s license, and Motor Vehicle report acceptable to the agency’s broker and verification of insurance coverage.
