Social Worker Discharge Planner

Note: Only candidates with strong skilled nursing discharge planning experience will be considered. This role is focused on managing complex discharge planning residents, ensuring safe, timely, and appropriate transitions of care. The ideal candidate is proactive, solutions-oriented, and comfortable navigating challenging discharge barriers while maintaining a high level of professionalism and accountability. Key Responsibilities Lead and coordinate individualized discharge plans for sub-acute and high‑acuity patients Proactively identify and resolve barriers to discharge (insurance, housing, social, clinical) Collaborate closely with interdisciplinary teams, patients, and families to drive timely outcomes Utilize strong knowledge of community resources, ALF/housing options, and alternative levels of care to secure appropriate placements Maintain accurate, compliant documentation in accordance with facility and regulatory standards Partner with admissions and leadership to support census strategy Manage complex cases with a focus on efficiency, safety, and patient‑centered outcomes Qualifications Bachelor’s or Master’s degree in Social Work or related field required Minimum of 3 years of discharge planning experience in a skilled nursing facility (required) Proven experience handling complex discharge cases (housing challenges, difficult placements, payer barriers) Strong knowledge of Medicare, Medicaid, Managed Care, and Long‑Term Care Insurance Deep familiarity with community‑based services, ALF/housing programs, and alternative levels of care Excellent communication, organization, and critical thinking skills Ability to work with urgency, manage competing priorities, and drive results Compensation & Benefits Salary starting commensurate with experience Medical and Dental PTO #J-18808-Ljbffr Dry Harbor Nursing Home and Rehabilitation Center
