Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0-810832, 40 hours per week, starting September 28th.

The Outpatient Physical Therapist evaluates all assigned patients, developing, implementing, and modifying treatment plans in accordance with the patient's needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. This role also includes educational responsibilities for patients, families, students, coworkers, and self.

Responsibilities:

• Conduct comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals to progress patients towards discharge.
• Complete medical record documentation in a clear, concise, accurate, and timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student programs by providing a positive learning environment.
• Actively pursue professional development through continuing education and self-study.
• Adhere to discipline-specific State Practice Act and maintain professional licensure.
• Understand organization's performance improvement methodology and contribute to quality service improvement.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license in good standing.
• Previous experience with our agency is highly desired.
