Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Identify and ensure cost-efficient, appropriate levels of care
• Coordinate with other professionals for appropriate patient care transitions
• Engage specialty resources as needed for optimal patient outcomes
• Maintain detailed records of outcomes during the management process
• Advocate for the patient, ensuring privacy and confidentiality

Requirements of the RN - Case Management - Travel

• Florida License required, compact license accepted
• Associates or Bachelor Degree in Nursing or related field
• Experience with OASIS/HCHB, wound care, PICC/IV care, and post-surgical care
• Ability to analyze complex medical information and make rational decisions
