Job Description Bridge Home Health & Hospice is seeking a skilled Physical Therapist who thrives in new environments and enjoys bringing clinical impact to communities across sunny California. This internal travel role offers guaranteed 40 hours per week, 13‑week assignments, housing and food stipend, full benefits, and the stability of a direct employee position. Candidates must be based in California or willing to relocate. Responsibilities Provides physical therapy services to patients according to a written physician's plan of care and the state Physical Therapy Practice Act. Assesses and evaluates therapeutic, rehabilitative, and functional status, and participates in the development of the total plan of care. Assesses muscle strength, mobility, gait, ROM, and potential for rehabilitation. Directs physical therapy treatment and instructs patients and caregivers in use and care of therapeutic appliances. Determines priority needs for physical therapy and reports patients' reactions to treatment or changes in condition to the physician. Initiates physical therapy program, instructs other personnel and/or family or caregiver members in certain phases of therapy, and participates in case conferences. May train patients in the use of prosthetic devices. Identifies patient and family/caregiver needs for other home health services and refers as necessary. Prepares and submits clinical and progress summaries based on attainment of goals. Provides physical therapy consultation to home families or caregivers when indicated. Provides in‑service education programs for nursing organization personnel as needed. Participates in peer consultation processes. Supervises physical therapy assistants according to organization policy and state regulations. When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two weeks. Qualifications Post‑baccalaureate degree in Physical Therapy (Doctor of Physical Therapy) approved by CAPTE. For PTs licensed prior to January 2016, a baccalaureate or master’s in PT from a program approved by CAPT and licensed by the Physical Therapy Board of California. Good verbal and written communication and organization skills. Current CPR certification. Valid auto insurance and current driver’s license with a vehicle in good working order. Must reside at least 55 miles from the work location and have reliable transportation. Physical Requirements Sit occasionally for 1‑3 hours. Stand frequently for 3‑6 hours. Drive/operate foot control occasionally for 1‑3 hours. Walk occasionally on level surface 1‑3 hours. Walk intermittently on unlevel surface for less than 1 hour. Bend or climb intermittently for less than 1 hour. Twist, kneel, crouch, and crawl occasionally 1‑3 hours. Lift, carry, push, or pull up to 50 pounds intermittently in less than 1 hour.
