Seven Healthcare are seeking a dedicated RN Hospice Case Manager for a travel assignment in Fort Worth, TX. This role specializes in providing hospice care and involves working collaboratively within an interdisciplinary team to ensure comprehensive patient care. The ideal candidate will have hospice experience and be proficient with HCHB.

Key Responsibilities of the RN - Hospice - Travel

• Identify and individualize the patient's and family's needs based on comprehensive assessments
• Document nursing assessments, goals, interventions, and outcomes
• Assess and manage patient's pain, anticipating and treating undesirable symptoms
• Participate in IDT meetings to review and revise care plans
• Maintain complete and accurate electronic medical records
• Educate patients, families, and other caregivers on interventions and medications
• Coordinate holistic, family-centered care in accordance with NHPCO standards

• RN license
• Hospice experience
• Proficiency with HCHB
