Case Manager

The Case Manager (CM) coordinates care to individuals living with HIV/AIDS. The CM provides a range of client-centered services to link clients with health care, psychosocial, substance use services, and other community services. Also providing support around timely medical treatment and coordination of continuity of care. The goal with each client is to obtain viral suppression through a multi-team approach. This position is part-time with an anticipated hours per week of 25 to 28 hours, that may change based on business needs. The pay range for this position is $24.45 to $32.90 per hour. Part-time employees accrue Earned Paid Leave time, and are eligible for benefits such as dental, vision, and life insurance; Employee Assistance Program (EAP) benefits, and wellness incentives.
