Physical Therapist Assistant

Job Summary:

We seek a highly skilled and experienced Registered Nurse to provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. The role involves patient assessments, medication administration, treatment planning, and collaboration with the interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are essential. A minimum of 2 years of experience and exceptional skill is required. Responsibilities:

• Perform comprehensive patient assessments and develop treatment plans.
• Administer medications and monitor patient responses.
• Provide skilled nursing care tailored to patient needs in medical/surgical, ICU, ER, psychiatric, labor & delivery, PACU, or operating room settings.
• Collaborate effectively with interdisciplinary healthcare teams to coordinate patient care.
• Manage case documentation and ensure compliance with healthcare regulations.
• Support hospice care patients and their families with compassionate care and communication.

Required Skills & Certifications:

• Active Registered Nurse license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum of 2 years experience in hospital, home health, or hospice care.
• Proficiency in clinical assessment, medication administration, and treatment planning.
• Strong interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Driver's license, car insurance, and reliable automobile for field work.

Preferred Skills & Certifications:

• Experience in specialized nursing areas such as ICU, ER, psychiatric, labor & delivery, PACU, or operating room.
• Additional certifications relevant to specialty areas.

Special Considerations:

• Ability to work in fast-paced and diverse healthcare environments.
• May require on-call availability depending on assignment.
• Compliance with all regulatory and facility-specific policies.

Scheduling:

• Day shifts with 40 hours per week, including holiday weeks.
• Overtime compensated at 1.5 times regular pay.
• Shift assignments and schedules may be provided during orientation or first day.
