Medical Social Worker

Job Description
We are seeking a Licensed Master Social Worker (LMSW) for an inpatient hospital-based assignment supporting medicine, surgery, and rehabilitation patients. The ideal candidate will have experience in psychosocial assessments, supportive counseling, discharge planning, and referral coordination within an acute care setting. This role requires strong communication skills, clinical judgment, and the ability to collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care and successful care transitions.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for inpatient patients and families.
- Provide supportive therapy and emotional support to patients experiencing medical, social, or behavioral challenges.
- Develop and coordinate discharge planning to ensure safe and effective patient transitions of care.
- Facilitate referrals to community resources, rehabilitation services, behavioral health programs, and post-acute care providers.
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to support patient treatment plans.
- Identify psychosocial barriers that may impact recovery, treatment adherence, or discharge outcomes.
- Advocate for patient needs and assist with care coordination throughout hospitalization.
- Maintain accurate and timely documentation in accordance with organizational and regulatory standards.
- Support patients and families with crisis intervention, coping strategies, and resource education.
- Participate in interdisciplinary meetings, patient care planning, and case discussions as needed.
Qualifications
- Master’s Degree in Social Work required.
- Current Licensed Master Social Worker (LMSW) license issued by NYSED required with primary source verification.
- Minimum of one (1) year of LMSW experience required.
- Hospital-based social work experience preferred/required.
- Experience in inpatient medicine, surgery (med/surg), and rehabilitation settings preferred.
- Strong experience with psychosocial assessments, supportive therapy, discharge planning, and referral management required.
- Excellent communication, interpersonal, and crisis intervention skills required.
- Ability to work collaboratively within interdisciplinary healthcare teams required.
- Strong organizational, documentation, and patient advocacy skills required.
- English proficiency required (reading, writing, and speaking).
- Availability to work day shifts from 9:00 AM – 5:00 PM required.
Important Details:
- 13-week contract position (extensions available)
- Shift is 8-hour day shift (9:00 AM – 5:00 PM), 40 hrs/week
- Location is New York, NY 10016
- Pay Range for Locals: $55 - $60 per hour
- Pay Range for Travelers: $2,350 - $2,400 per week
