Registered Nurse (RN) Case Manager – Senior Care

Location: Carson, CA
Pay: $50.00–$54.00 per hour
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Job Type: Full-Time Contract | 13-Week Assignment | 40 Hours/Week
Setting: Hybrid Clinic & Field-Based/Home Visits

We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join a collaborative care team serving frail and elderly adults in Carson, CA.

This is an excellent opportunity for an RN with strong clinical assessment and care coordination experience who enjoys developing meaningful relationships with older adults. You will manage an assigned patient panel and work closely with Primary Care Providers and an interdisciplinary team to coordinate comprehensive, patient-centered care.

What You'll Do

• Manage care and clinical outcomes for an assigned panel of patients.

• Conduct comprehensive nursing assessments in the clinic, virtually, and during home visits.

• Develop, implement, and update individualized Plans of Care.

• Coordinate care with physicians, interdisciplinary team members, patients, and families.

• Educate patients and families regarding acute, chronic, and end-stage conditions.

• Monitor changes in patient condition and escalate clinical concerns appropriately.

• Coordinate transitions between hospitals, emergency departments, skilled nursing facilities, hospice, clinics, and home settings.

• Help prevent unnecessary hospitalizations through proactive care coordination and follow-up.

• Coordinate services including specialists, labs, procedures, personal care, dental, vision, and transportation.

• Process and follow up on medical orders, test results, and clinical communications.

• Perform nursing procedures and provide direct patient care when needed.

• Complete accurate and timely documentation in the Electronic Health Record (EHR).

• Conduct local home visits as required.

Required Qualifications

• Graduate of an accredited School of Nursing.

• Active, unrestricted Registered Nurse (RN) license.

• Active BLS certification.

• Minimum 2 years of clinical nursing experience caring for frail or elderly adults.

• Strong clinical assessment and decision-making skills.

• Experience coordinating care for patients with complex healthcare needs.

• Strong organizational, communication, and time-management skills.

• Ability to work independently and make appropriate clinical decisions.

• Comfortable working with patients from diverse cultural and socioeconomic backgrounds.

• Ability and willingness to conduct local home visits.

Preferred

• Bilingual English/Spanish verbal and written communication skills.

• Previous RN case management, care coordination, geriatric, home health, skilled nursing, PACE, or similar senior-care experience.

Position Details

Pay: $50.00–$54.00/hour
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours: 40 hours per week
Contract Length: 13 weeks
Location: Carson, CA
Work Setting: Hybrid clinic and field-based position

Local travel may be required approximately 2–5 times per week for home visits within the surrounding Carson area.
