PRN - Physical Therapist

Graduate of an accredited Physical Therapy School. Licensed in the State of Illinois. Current CPR card Knowledge of general P.T. practice act. Knowledge of Rehab/PT policy and procedures. Capable of communicating effectively verbally or in writing. Must have good leadership skills. Description: Pay Scale: $60.00 per hour Evaluates and records patient treatment plan in coordination with their physician. Establishes patient treatment programs to include modalities of heat, cold, massage, mobilization, hydrotherapy, light therapy, electricity and patient education as per orders from the physician. Evaluates gait patterns and instructs patients in proper technique and use of devices. Coordinates patient care or treatment with other members of the team according to abilities. Adapts treatments according to the level of improvement of the patient. Provides home health services through a contract with Addus or Egyptian Health Department. Performs other duties as required or assigned.
