What You’ll Do Perform medication management support Set up and assist with medication organization Serve as a liaison between clients and physicians Complete patient assessments and accurate documentation Supervise Personal Care Assistants (PCAs) Support coordinated, patient-centered care plans Ensure continuity of care and communication across providers What We’re Looking For Active Registered Nurse (RN) license required Strong assessment and clinical judgment skills Highly organized and detail-oriented Ability to work independently in the field Strong communication and interpersonal skills Compassionate, patient-focused approach to care What We Offer Flexible weekday schedule (8:00 AM – 4:30 PM) Opportunity to build your own schedule Competitive wages and benefits Training provided Meaningful, one-on-one patient care Supportive team environment New Dimensions Home Health Care is dedicated to helping individuals live safely and independently in their own homes.
