Health Services Coordinator Pay Range: $34 - $36/hr Expected Weekly Hours: 24 Schedule: Sunday - Tuesday Job Summary The Health Services Coordinator serves on the health services team, supporting the department's overall operations through a variety of administrative and clinical functions as directed by the Director of Health Services (DHS). The Coordinator preserves dignity and promotes independence for each resident while providing care and services in accordance with each service plan and MBK policy and procedure. Duties & Responsibilities Conduct and coordinate assessments/evaluations of potential residents and make recommendations for admission in accordance with current rules, regulations, and community policies. Draft initial Individualized Service Plans (ISP) and update as needed. Review service plans with the responsible parties and the Executive Director as requested by DHS. Ensure continuity of the assisted living residents’ total care regimen. Provide training and education on topics such as medication delivery, treatments, safety protocols, emergency procedures, accurate records, and state requirements. Perform all assigned duties accurately and timely, including required documentation. Serve as medication technician as needed. Provide coverage of job duties within the department during absences. Check vital signs as directed and monitor health changes. Perform basic nursing functions such as treatments, medication delivery, and managing resident emergencies. Maintain adequate inventory of resident care supplies and minimize waste. Coordinate prescription orders with doctors’ offices and manage pharmacy delivery, oversee central storage, tracking, and delivery of medications, and ensure Medication Administration Records are completed per policy and regulations. Aid in community marketing through positive interactions and liaison to families and outside providers. Maintain a safe and secure environment for all staff, residents, and guests following established safety standards, policies, and procedures. Promote teamwork, open communication, and align with MBK principles and core values. Perform other job duties or special projects as assigned. Education Requirements RN, LVN, or LPN license that is active and in good standing is essential, including adherence to all requirements to maintain license, CEU completion, and timely renewal. Experience Requirements 2+ years of prior related work experience in a similar healthcare environment. Required Competencies / Licenses / Certifications Complete required background clearances, health screening, and provide negative TB test results within 7 days of employment (must be within the last 6 months). Valid state driver’s license and reliable transportation. Competent and current technical and computer skills, including familiarity with Microsoft Office Suite (Word, Excel, Outlook, etc.) and office equipment. Excellent communication skills, including speaking, writing, and reading English. Ability to make sound, independent decisions when circumstances warrant, and remain calm during stressful or emergency situations. Ability to deal tactfully and professionally with personnel, residents, family members, and guests. Strong organizational, problem‑solving, and time‑management skills. Maintain a neat and organized work environment to promote safe coordination of resident care. Physical Demands & Work Environment Must be able to work a flexible schedule, including weekends and holidays. Must be mobile and able to perform walking, bending, kneeling, squatting, pulling, reaching overhead, and repetitive motion. Ability to lift and carry up to 50 pounds and push up to 250 pounds as necessary. Ability to assist in the physical movement of residents during routine transfers or emergencies.
