Overview

This position is a temporary contract role within an acute care facility, offering an exciting opportunity to contribute to patient care and management.

Position Details

• Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)
• Start Date: As soon as possible
• Location: City, State
• Floating Expectation: None required
• First-Time Traveler: Not accepted; seasoned travelers preferred

Job Requirements

• Years of Experience Required: Minimum of 2 years in acute care case management/discharge planning
• Patient Ratios: Days 1:25, Weekends 1:45
• License: California state license required; pending licenses accepted if obtaining licensure by start date
• RTO Restrictions: A maximum of 7 days

Qualifications

• Must have hospital experience; behavioral health experience is not applicable.
• Experience with assessing patients, care coordination, discharge planning, and utilization review is required.
• Ability to organize and participate in family meetings.
• Knowledge of health maintenance organizations (HMOs), independent physician associations (IPAs), and other related systems.
• Competence in arranging durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport.

Unit Details

This role involves working with a diverse patient population, including neonates, adolescents, adults, and geriatrics, with an average daily census of 25 patients. The facility provides interdisciplinary support from physical therapy and social services.

Special Procedures/Unit Information

• Experience with patients facing various challenges, including homelessness, mental health crises, and substance abuse issues, is beneficial.
• Competence with computerized documentation is mandatory.
• Expectations include participation in the treatment plan development and execution.

Additional Notes

Candidates will be expected to float between nearby facilities if required for the assignment. Refusal to float may result in termination of the assignment.

All requested time off and scheduling preferences must be made at the time of submission and cannot be adjusted post-offer. Flexibility beyond the contracted schedule may lead to termination of the contract.
