  • Conduct thorough evaluations and psychosocial assessments to identify the needs of seniors and their families.

  • Develop and implement individualized care plans, liaising with other professionals to coordinate and enhance service delivery.

Patient Population / Client Focus\n

You will primarily work with elderly individuals in a hospice setting—many of whom are battling terminal illnesses—and their families. Your expertise in understanding the psychosocial aspects of aging will be vital in helping them cope with end-of-life issues. You will also need to be sensitive to the diverse backgrounds and cultures of our clients, tailoring your approach to meet their unique needs.

Qualifications\n

  • Master's degree in Social Work (MSW) or a closely related field.

  • Current licensure as a Licensed Clinical Social Worker (LCSW) in Texas.

  • Expert understanding of assessment, diagnosis, and treatment methodologies pertinent to geriatric populations.

  • Strong verbal and written communication skills, allowing for effective interactions with clients, families, and team members.

  • Proven ability to manage complex cases and navigate challenging emotional discussions.

Preferred Experience\n

  • A minimum of two years of experience providing clinical social work services, particularly in hospice or palliative care settings.

  • Experience in developing and implementing Individual Family Service Plans (IFSP) or similar care models.

  • Bilingual proficiency in English and Spanish is highly preferred to better serve our diverse community.
