Hospice Admission Nurse

Overview Elara Caring is seeking an Admission Nurse to support growth in the home care industry. Each employee has the opportunity to make a real difference by carrying out our mission every day. Responsibilities Promote Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care. Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. Assess assigned visits of hospice patients to identify physical, psychosocial, and environmental needs as documented in the clinical record and admission paperwork. Participate in the implementation of the plan of care for an assigned caseload to ensure quality, continuity of care, and achievement of patient outcomes. Make the initial evaluation and re-evaluate the patient’s nursing needs during each visit. Communicate significant findings, problems, and changes in condition or environment to the Patient Care Manager, the physician, and/or other personnel involved with patient care within 24 hours. Report unsafe conditions to the appropriate Clinical Supervisor and physician in a timely manner. Identify deviations from normal nursing and patient condition status, revise the plan of care and initiate appropriate actions based on the deviation. Perform other duties/projects as assigned. Qualifications Graduate of an accredited nursing program. Current, unrestricted state RN licensee. One-year experience in a clinical care setting as a nurse. Able and willing to travel within branch/office coverage area and 50% of the time. Must have a dependable vehicle, valid driver’s license and current auto insurance in accordance with state laws. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. Associate’s Degree or Bachelor’s Degree in Nursing preferred.
