Nurse Navigator Oncology

Nurse NavigatorShift: M-F 8:00am- 4:30pmSinai Hospital is seeking a compassionate and detail-oriented Nurse Navigator to join our Medical Oncology team. This vital role is responsible for guiding patients through the complex oncology care continuum-from diagnosis through survivorship-by ensuring seamless coordination, communication, education, and support. The Nurse Navigator works closely with physicians, nurse practitioners, and other interdisciplinary team members to proactively identify high-risk patients, coordinate care, and improve outcomes through patient-centered navigation services.Key Responsibilities:Care Coordination & Patient Navigation:Identify high-risk oncology patients and provide proactive outreach and follow-up, including in-person and telephone communication.Collaborate with physicians and care teams to develop individualized care plans, coordinate follow-up appointments, scans, and diagnostic tests.Ensure timely communication and coordination between the patient, family, and care team, acting as a primary point of contact.Serve as a liaison between referring physicians and the oncology team to ensure streamlined transitions in care.Monitor treatment progress, compliance, and follow-up needs; support patients in understanding their treatment plans and care pathways.Coordinate medication refills, imaging, lab orders, and necessary authorizations in partnership with the provider and financial coordinator.Patient Education & Support:Provide anticipatory guidance and education for patients with chronic diseases or new diagnoses, emphasizing self-management and care ownership.Teach patients and families how to schedule scans, arrange transportation, and navigate the healthcare system.Integrate patient and family education throughout all phases of care, adapting to individual learning needs and cultural considerations.Clinical Support & Communication:Triage abnormal lab results and clinical messages for providers.Manage prescription refill requests and clinical inquiries.Support providers in addressing urgent patient issues and care coordination.Program Development & Quality Improvement:Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.Contribute to accreditation efforts and the ongoing development of the cancer program through data collection, education, and process improvement.Maintain awareness of and adherence to regulatory requirements, hospital policies, and best practices in oncology nursing.Qualifications:Required Education:Bachelor of Science in Nursing (BSN)Licenses & Certifications:Registered Nurse (RN) - Current Maryland license or eligibility to obtainBasic Life Support (BLS) - Current certificationPreferred:Oncology Certified Nurse (OCN) certification strongly preferredSkills & Attributes:Strong knowledge of oncology care and treatment protocolsExcellent communication and interpersonal skills with patients, families, and healthcare professionalsAbility to manage multiple priorities, coordinate care across disciplines, and provide empathetic supportWorking knowledge of healthcare systems, insurance, and community resourcesBenefits:Medical, dental, and vision coverage starting the first of the month after hire, plus wellness programs and mental health support403(b) retirement plan with employer match, pension, and life and disability insuranceTuition reimbursement toward advanced degrees and specialty certificationsGenerous paid time off, fitness discounts, and free on-site parking
