Case Manager - OPD - FT

Mount Carmel Guild Behavioral Health System, a CARF accredited division of Catholic Charities of the Archdiocese of Newark, is a leading provider of mental health and addiction services in Northern New Jersey.

We are currently seeking a  Full Time Case Manager for our  Outpatient Department in Jersey City, NJ . Our  Outpatient Program uses a multi-disciplinary approach; emphasis is placed on individualized assessment, goal-oriented treatment planning, periodic case review, and continuity of care for all persons served.

Job Description:

• Function as part of OPD team
• Interfaces with OPD psychiatrist
• Assists staff psychiatrist with scribing clinical assessments
• Completes treatment plans and maintains staff psychiatrist caseload of clients
• Completes documentation according to agency and DMHAS standars

Job Requirements:

• Requires bachelor's degree in a human services field
