Responsibilities:

• Evaluate and assess patients physical therapy needs
• Develop individualized treatment plans
• Provide therapeutic exercises and rehabilitation services
• Educate patients and families on treatment plans and mobility improvement
• Coordinate care with physicians and interdisciplinary team members
• Complete timely and accurate documentation

Qualifications:

• Active Colorado Physical Therapist (PT) license
• Home Health experience preferred, but not required
• Strong communication and patient care skills
• Reliable transportation preferred

Competitive Pay ($75$130/hour). Pay rates vary by type of visit.
