Responsible for using EPIC reports to identify ED patients that were discharged.2. Responsible for calling discharged patients to follow up on their visit and provide any necessary clarification of discharge instructions.3. Provides service recovery as needed.4. Calls to be made within 48 hours of patient discharge.QualificationsEDUCATIONGraduate of an accredited school of nursing (Diploma, AD, BS, BSN)EXPERIENCE2 years ED experienceLICENSURERN In State of CT or Permit; Basic Life Support (BLS)SPECIAL SKILLSComputer skills, strong service excellence telephone skillsPHYSICAL DEMANDMay be exposed to infectious diseases.
