Staff Nurse II - Practice Plan-(Family Medicine)

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our dynamic clinical care team. In this role, you will provide direct patient care, manage triage services, and support a collaborative team of 12 providers, 2 LVNs, 6 MAs, and additional clinical staff. Your day-to-day responsibilities will include assessing and implementing care plans for patients with both routine and complex needs, responding to patient concerns both in-person and by phone, and ensuring seamless coordination among care team members. Ideal candidates will bring experience in chronic disease management, triage, care coordination, and a commitment to quality improvement initiatives. Proficiency with Epic or other EMR systems, as well as experience mentoring or supervising LVNs, MAs, or nursing students, will be highly valued.
