This 13-week travel PT assignment offers hands-on, patient-centered in-home therapy in a Medicare-certified home health setting.


Job Details

• Weekly Estimated Pay: $2,310 - $2,480
• Location: Mission Viejo, CA - serving surrounding Orange County communities
• Employment Type: Travel Contract (Travel Physical Therapist / Travel PT)
• Duration: 13 weeks
• Schedule: Tuesday - Saturday, 8:00 AM - 5:00 PM
• Guaranteed Hours: 40 hours per week
• Shift: 8-hour days
• On-call/Weekend Requirements: Yes (rotational)
• Productivity: 30 points per week
• EMR: HCHB (HomeCare HomeBase)

Job Requirements / Qualifications

• Active Physical Therapist (PT) license in California (required)
• Minimum 1 year of home health experience in a Medicare-certified home health agency
• Proficiency with HCHB electronic medical records (documentation experience preferred)
• Ability to travel within the assigned territory for in-home visits
• Strong assessment, treatment planning, and clinical documentation skills focused on home health physical therapy
• Excellent communication and collaboration skills for interdisciplinary care coordination

Primary Responsibilities

• Provide high-quality in-home physical therapy services to adult and geriatric patients across the assigned territory
• Perform comprehensive evaluations and develop individualized treatment plans and measurable goals
• Deliver skilled interventions focused on mobility, balance, fall prevention, strength, and ADL training in the home environment
• Maintain accurate, timely documentation in HCHB EMR to meet Medicare and agency standards
• Collaborate with nurses, OTs, SLPs, physicians, and case managers to ensure coordinated, patient-centered care
• Meet productivity expectations (30 points per week) and participate in on-call/weekend coverage as assigned
