Responsibilities
- Perform skilled physical therapy evaluations and develop individualized plans of care for home health patients
- Deliver therapeutic interventions focused on gait, balance, strength, ROM, and activities of daily living
- Monitor patient progress, update treatment plans, and document outcomes in the electronic medical record
- Conduct Start of Care visits and periodic revisits, adjusting plans as needed to meet patient goals
- Educate patients, families, and caregivers on safety, transfers, and home exercise programs
- Collaborate with the interdisciplinary team to coordinate care and ensure smooth transitions and discharge planning

Requirements
- Active Physical Therapist license in Pennsylvania or eligibility for PA licensure
- Recent home health experience
- EPIC EMR experience and proficiency with OASIS documentation
- Demonstrated ability to perform Start of Care and revisits in a home health setting
- Ability to commit to 40 hours per week on a 5x8 daytime schedule
- Willingness to travel within the assignment area to provide home visits
