Hospice Resource RN - NOC Shift (Dodgville Area)

Job Details Location: Dodgeville, WI 53533 Position Type: Part Time Travel: Driving Shift: NOC Hours: 12:00 AM – 8:30 AM (Weekdays) FTE: 0.7 (28 hours/week) Includes on‑call rotation (1 NOC Shift/Month), weekend rotation (every other weekend), and holiday rotation (3/year) Sign‑On Bonus: $10,000 About Agrace Nationally recognized nonprofit leader in hospice and palliative care, serving families across Wisconsin. Key Responsibilities Patient and Family Assessments • Assess patients and families for physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs. • Collect data from patients, families, and interdisciplinary team members. • Identify, interpret, and document assessment data. • Report significant changes or incidents to appropriate IDT member. • Collaborate with the IDT and primary care provider to create HES for hospice eligibility. Patient Visits • Respond to patient and family needs, including acute interventions. • Provide pain management and treatment.• Evaluate effectiveness of pain management or other interventions and adjust accordingly. • Educate patients and families on pain, symptom management, and end‑of‑life care. Plan of Care • Formulate, implement, and revise individualized POC with IDT, patient, and family. • Set measurable outcome goals and priorities. • Follow and document the POC in the electronic health record. • Delegate and manage visits for CNA and LPN staff. • Oversee work of Certified Nursing Assistants (CNA) and Licensed Practical Nurses (LPN). Miscellaneous • Participate in meetings to plan and evaluate patient/family services. • Precept new staff and students. Qualifications Current, unrestricted Registered Nursing License in the state of employment. One year of nursing experience preferred. Valid driver’s license, auto insurance, and reliable vehicle. Minimum age 18 years. Basic Microsoft Office skills. Intermediate electronic health record skills. Proficiency in proper body mechanics, safe patient handling, and standard precautions.
