Responsibilities
- Provide direct and indirect skilled nursing care to adult and geriatric residents under RN supervision
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the interdisciplinary team
- Contribute to development and modification of an age-appropriate plan of care, involving residents and families
- Implement and revise nursing interventions to achieve resident outcomes; coordinate referrals as needed
- Maintain current nursing knowledge and participate in continuing education and inservice programs
- Demonstrate leadership within the professional practice setting and uphold safety standards for residents and staff

Requirements
- Active Licensed Practical Nurse license in New York
- BLS certification (American Heart Association or RQI-compliant)
- Minimum 1 year of nursing experience as an LPN, with prior LTC exposure preferred
- Ability to provide care to adult and geriatric residents and to work the evening shift
- Knowledge of growth and development across the adult to geriatric lifespan
- Ability to work within a multidisciplinary team and follow established policies and procedures
