RN, Hospice (PT, Home-Based, M-TH, 4:30pm-8am)

Under the supervision of the Clinical Director, the Registered Nurse oversees the primary caseload of hospice patients and provides skilled nursing care to patients, their families, and caregivers as prescribed by the physician and in compliance with hospice Medicare Conditions of Participation, Pennsylvania hospice licensure laws, and agency policies and procedures. The RN conducts the initial assessment and comprehensive nursing assessment of patient needs, collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care, implements the plan, and reevaluates and revises it relative to patient needs to provide appropriate, comprehensive, and responsive goals and interventions. The RN works closely with other members of the IDT to ensure all patient and family care needs are being met and provides supervision to Hospice Aides/Homemakers. Responsibilities Oversee primary caseload of hospice patients and provide skilled nursing care. Conduct initial and comprehensive nursing assessments of patient needs. Collaborate to develop and implement the hospice IDT plan of care. Reevaluate and revise the plan of care relative to patient nursing needs. Work closely with other IDT members to meet all patient and family care needs. Provide supervision to hospice aides/homemakers. Qualifications Graduate of an accredited nursing program required. Bachelor’s degree in nursing preferred. Certified Hospice and Palliative Nurse (CHPN) certification preferred. Minimum of one year general nursing experience; current hospice, home health, medical, surgical, or critical care experience preferred. Experience with pain and symptom management interventions preferred. Ability to work within an interdisciplinary setting. Understanding of hospice philosophy and principles of death and dying. Special Employment Requirements Current RN license issued by the Pennsylvania State Board of Nurse Examiners required. Clearance by PA State Police criminal history check required. Valid PA driver’s license and available transportation during all working hours required. Maintains regular attendance. Functions independently but seeks and accepts guidance from other IDT members and community members as warranted. Conducts all business activities in a professional and ethical manner. Demonstrates highest degree of customer awareness and genuine concern, empathy, and optimism. Communicates effectively and courteously with patients, caregivers, colleagues, supervisors, and co‑workers. Maintains confidentiality of patient information. Is proficient with manual dexterity for writing, keyboarding, and telephone operation. Manages stress efficiently. Demonstrates respectful treatment of each person with dignity. Working Conditions Works in a well-lit office environment and in varying conditions in patient homes, which may be less than desirable.
