- At least 1 year of recent nursing experience required, with case management experience preferred
- Active Registered Nurse (RN) license in the state of Maryland or compact state eligibility, as applicable
- Strong knowledge of care coordination , discharge planning , and utilization review
- Ability to communicate effectively with patients, families, physicians, and interdisciplinary healthcare teams
- Excellent organizational skills and attention to detail
- Current BLS certification preferred
- Coordinate patient care plans across the continuum of care to support safe, effective treatment and discharge outcomes
- Collaborate with physicians, nursing staff, social services, and ancillary departments to assess patient needs
- Facilitate discharge planning and connect patients with appropriate community resources and follow-up services
- Monitor patient progress and evaluate care plans for quality, efficiency, and medical necessity
- Advocate for patients and families by helping them understand treatment plans, available services, and next steps
- Maintain accurate and timely documentation in accordance with facility and regulatory standards
