Case Management RN Requirements

At least 1 year of recent nursing experience required, with case management experience preferred
Active Registered Nurse (RN) license in the state of Maryland or compact state eligibility, as applicable
Strong knowledge of care coordination , discharge planning , and utilization review
Ability to communicate effectively with patients, families, physicians, and interdisciplinary healthcare teams
Excellent organizational skills and attention to detail
Current BLS certification preferred

Responsibilities

Coordinate patient care plans across the continuum of care to support safe, effective treatment and discharge outcomes
Collaborate with physicians, nursing staff, social services, and ancillary departments to assess patient needs
Facilitate discharge planning and connect patients with appropriate community resources and follow-up services
Monitor patient progress and evaluate care plans for quality, efficiency, and medical necessity
Advocate for patients and families by helping them understand treatment plans, available services, and next steps
Maintain accurate and timely documentation in accordance with facility and regulatory standards
