Primary Care Physician

- Provide comprehensive outpatient care to adult and senior patients
- Diagnose and manage complex medical conditions
- Maintain accurate documentation and coordinate necessary referrals
- Collaborate with an interdisciplinary care team
- Participate in quality improvement and clinical initiatives
- MD or DO; active, unrestricted medical license
- Board Certified in Family Medicine, Internal Medicine, or Geriatric Medicine
- Minimum 2 years of experience in value‑based care or high‑acuity geriatric populations
- Commitment to patient‑centered, outcomes‑driven care
- Participation in TB screening
- Training in Family Medicine, Internal Medicine, Med‑Peds, or Geriatrics
- Experience in senior care or value‑based care settings
- Proficiency with EHR systems
- Strong communication and team collaboration skills
- Experience supervising NPs/PAs
- Clinic Hours are Monday-Friday (8a - 5p), this position is 32 hours a week
