Job Description:

We are seeking an experienced Registered Nurse Care Manager to support its Inpatient Care Management team. 

The ideal candidate will have a strong background in Care Coordination, Utilization Review, and Discharge Planning within an acute care hospital setting.

Key Responsibilities:

• Coordinate inpatient care management activities.
• Perform utilization review and ensure appropriate level of care.
• Develop and facilitate discharge plans for patients.
• Collaborate with interdisciplinary healthcare teams.
• Maintain accurate documentation and care management records.
• Support patient-centered care and resource utilization initiatives.

Must-Have Qualifications: ✔ Active RN License in the State of New York ✔ Associate Degree from an accredited Nursing Program accepted per facility requirements ✔ Minimum 5 years of clinical experience as a Registered Nurse ✔ Inpatient Care Management experience (IP experience required; ED-only experience not preferred) ✔ Experience with Care Coordination, Utilization Review, and Discharge Planning ✔ Current BLS Certification ✔ NYSED Primary Source Verification of RN License and Registration ✔ PRI & Screen Certification (must be available at submission or obtainable per facility requirements)
