Therapy Services Specialist Sr. - CareBridge

The Therapy Services Specialist Sr. is responsible for working directly to support new and current specialist.

Location: Virtual - This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.

How you will make an impact:

• Work with the lead case manager on state specific processes and policies and ensure that state specific standard operating procedures are up to date.
• May partner with leadership and assists in leading procedure and process updates.
• Performs telephonic and/or virtual assessments to identify participants needs on more complex cases.
• Support leads in overseeing recommendations to MCO for type and hours of supportive services required.
• Support process to conduct objective assessments for program participation to determine the appropriate level of support and services required.
• Support lead in the overseeing the assessments annually or more frequently as needed in accordance with applicable program requirements and participants needs.
• Assists with case consults.
• Mentor and provide shadowing opportunities during new hire orientation or on an ongoing basis. May serve as the back up to lead associates.
• Assists leads with CFER reviews.

Minimum Requirements:

• Requires graduate of a college level program in physical therapy, occupational therapy, or an accredited two-year program for a Physical Therapist Assistant or Occupational Therapist Assistant and minimum of 3+ years' experience in social service or health care field; or any combination of education and experience, which would provide an equivalent background.
• Current active, valid and unrestricted license or certification as a physical therapist assistant or occupational therapist assistant in applicable state required. Certification and/or licensure appropriate to field of specialty as required.
