You’ll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient.**Key Responsibilities:*** Implement nursing care based on comprehensive assessments and care plans* Accurately document patient services to reflect the plan of care* Educate patients and families on diagnoses and self-care plans* Coordinate hospice services with the interdisciplinary team* Use equipment and supplies effectively and efficiently* Provide pain and symptom management, evaluating patient response* Participate in quality improvement and professional development activities**Qualifications:*** Valid California LVN license* Minimum 1 year of hospice experience* Experience with Home Care Home Base (HCHB) preferred* Current CPR certification* Valid driver’s license and insured, reliable vehicle**About Seaport Hospice**Seaport Hospice is part of the **Pennant Group**, a nationwide network with over 300 locations.
