Licensed Practical Nurse (LPN) Job Responsibilities include: Admissions, discharge, and transfers of residents Medication requisitions with pharmacy and physicians Medication administration Following infection control, emergencies, and other center policies Collaborating with resident’s interdisciplinary team to provide best outcomes Identifying and reporting all abnormal changes in residents’ condition per center policy Works under direct supervision in accordance with state regulations and facility policies Delivers nursing care to patients/residents requiring long‑term or rehabilitative care Collects patient/resident data, makes observations, and reports pertinent information Implements the patient/resident plan of care and evaluates the response per regulations Directs and supervises care given by other nursing personnel in selected situations Maintains knowledge of documentation requirements Maintains knowledge of equipment set‑up, maintenance, and use (e.g., monitors, infusion devices, drains) Maintains confidentiality and patient/resident rights regarding all information Licensed Practical Nurse (LPN) Qualifications: LPN licensed by the State CPR certification required Two years prior nursing experience preferred #J-18808-Ljbffr
