Renal Dietitian

The Renal Dietitian position is for our Central Valdosta Clinic, located at 506 N Patterson St, Valdosta, GA 31601, and our Adel Clinic, located at 701 N Hutchinson Ave, Adel, GA 31620. How you'll change lives As a Dietitian at U.S. Renal Care, you will be an integral part of an interdisciplinary team, providing excellent care for patients living with kidney disease. U.S. Renal care dietitians work under the supervision of a RDN Regional Manager. What you'll be doing Patient Nutrition. Using the most current, science-based practices, you will recommend therapeutic diets based on patient nutritional needs, preferences, and changes in treatment, with the patient’s physician. You will review lab results and provide appropriate education for nutritional interventions on fluid and sodium, potassium, and CKD-Mineral and Bone Disorder management. You will also conduct patient assessments, care planning activities, and participate in monthly interdisciplinary patient care conferences. Teamwork. Collaborate with the Medical Director, physician/advanced practice provider, facility administrator, social worker, nurse, and patient care technician to maintain positive relationships with area hospitals, skilled nursing facilities, agencies, vendors, and the community. Safety & Quality. Help with clinical and operational processes, focusing on improving patient health and achieving target goals for patient outcomes. Maintain a productive quality assurance program and participate in monthly Quality Assessment and Performance Improvement (QAPI) activities. #J-18808-Ljbffr
