Bilingual Virtual Nurse Practitioner — Telemedicine & Chronic Care

Elevance Health's CareBridge Health seeks a bilingual Nurse Practitioner for a 100% virtual role, collaborating with physicians and families to develop complex care plans for home-based patients. The NP will deliver urgent care via telehealth, create treatment plans, close gaps in care, educate patients, and maintain compliant records.
