Job Summary
Long Term Care (LTC) Nurse specializing in Skilled Nursing located in a facility providing subacute and post-acute care. This role involves supporting patients with pulmonary diagnoses requiring oxygen, Alzheimer's, and dementia, and involves performing tasks such as medication administration, wound care, trach care, and IV procedures. The position supports an interdisciplinary team in a dynamic, hospital-like setting.

Job Details

• Shift: Days, 12-hour shifts, with a minimum of 36 hours per week.
• Required Qualifications: Valid licensure to practice as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Certified Nursing Assistant (CNA), along with BLS for RNs, LPNs, and CNAs, ACLS (American Heart Association), and NIHSS (American Heart Association or APEX). Minimum of 2 years of relevant experience required.
• Preferred Qualifications: Prior experience in skilled nursing facilities or subacute care.
• Responsibilities: Support patients with pulmonary, Alzheimers, and dementia diagnoses; perform medication passes, wound care, trach care, IV management; utilize respiratory therapy equipment; participate in interdisciplinary team conferences.
• Certifications: ACLS (American Heart Association), BLS (American Heart Association), NIHSS (American Heart Association or APEX).
• Schedule: 12-hour days, every other weekend, may support acute rehab and medical-surgical/telemetry units, with responsibilities including coverage during holidays.
• Submission Requirements: Valid license to practice required for credentialing.
