Occupational Therapist (OT) – Part-Time & Per Diem Continental Springs 3200 G St, South Sioux City, NE 68776 Join Continental Springs and Make a Meaningful Impact At Continental Springs, our rehab team is dedicated to helping residents achieve their highest level of function, independence, and quality of life in a subacute/skilled nursing setting. We are seeking a compassionate and motivated Occupational Therapist to join our interdisciplinary team in a flexible part-time or per diem role. Responsibilities Evaluate residents’ functional abilities, including ADLs, mobility, cognition, coordination, and safety Develop and implement individualized occupational therapy treatment plans Provide therapeutic interventions to improve independence with daily activities and functional mobility Address deficits related to strength, range of motion, balance, coordination, cognition, and fine motor skills Recommend and train residents in the use of adaptive equipment and compensatory strategies Educate residents, caregivers, and staff to promote safety, independence, and carryover of therapy goals Collaborate with PT, SLP, nursing, physicians, and other members of the interdisciplinary team to support positive patient outcomes Monitor resident progress and modify treatment plans as clinically appropriate Maintain timely, accurate, and compliant clinical documentation Participate in care planning and interdisciplinary team communication Qualifications Active Occupational Therapist license in Nebraska (or eligibility for licensure) Graduate degree in Occupational Therapy from an accredited program NBCOT certification preferred New graduates and experienced therapists are welcome to apply Experience in skilled nursing or subacute rehabilitation preferred Strong communication, clinical reasoning, organization, and patient-centered skills What We Offer Flexible part-time and per diem scheduling Supportive interdisciplinary rehabilitation team Consistent facility-based patient population Collaborative clinical environment Competitive per diem compensation Opportunity to make a meaningful difference in residents’ independence and quality of life #J-18808-Ljbffr
