Field Outreach Case Manager - Homeless Services

Columbus House, Inc. in Middletown Township, PA seeks an Outreach Case Manager to engage adults experiencing homelessness, link them to services, support sobriety, and help stabilize their lives. This role focuses on direct street outreach, case management, and coordinating with partner agencies to provide comprehensive support.
