Team 11: Arverne, Astoria, Bayside, Belle Harbor , Bellerose, Breezy Point, Broad Channel, Cambria Heights, College Point, Corona , Douglastan, East Elmhurst, Far Rockaway, Floral Park, Flushing, Flower Hill, Forest Hills, Fresh Meadows, Glendale , Glen Oaks, Great Neck, Hollis, Howard Beach, Jackson Heights, Jamaica , Kew Gardens, Laurelton, Kings Point , Manhasset, Maspeth, Middle Village, New Cassel, Oakland Gardens , Ozone Park, Plandome , Queens Village, Rego Park, Richmond Hill, Ridgewood, & Rockaway Park.

• 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
• Per visits are required to attend an entire first week of orientation Monday - Friday (8:30AM -4:30PM.)
• Per Visits are required to work 1 shift per pay period.

Job Details

Our Home Care RN's:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications:

• New York State RN license
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Driver's License and reliable transportation

Posted Salary Range

USD $35.00 - USD $135.00 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
