Millenia Hospice is seeking a dependable, patient-centered, and professionally licensed Hospice Social Worker to join our interdisciplinary hospice team. In this meaningful role, you will provide counseling, education, resource coordination, crisis intervention, and emotional support to patients and families navigating terminal illness and end-of-life care.

You will work closely with the RN Case Manager, Medical Director, Chaplain, Hospice Aide, Administrator, and other members of the Interdisciplinary Group (IDG) to ensure that every patient’s Plan of Care reflects their goals, family dynamics, psychosocial needs, safety concerns, and end-of-life preferences.

Position Summary
The Hospice Social Worker assesses and addresses the psychosocial, emotional, financial, environmental, cultural, and caregiver-related needs of hospice patients and their families.

The ideal candidate is compassionate, organized, professional, and comfortable working both independently and collaboratively. This individual must also be able to complete clear, timely, and accurate electronic medical record documentation that reflects the patient’s psychosocial needs, caregiver burden, continued hospice eligibility, and “Why Hospice Now.”

Key Responsibilities

Psychosocial Assessment

• Complete initial and ongoing psychosocial assessments for hospice patients and their families.
• Assess emotional, social, spiritual, financial, environmental, cultural, and caregiver-related needs.
• Identify family dynamics, available support systems, caregiver stress, barriers to care, safety concerns, and the patient’s and family’s ability to cope.

Counseling and Emotional Support

• Provide supportive counseling related to terminal illness, grief, anticipatory grief, caregiver stress, loss, end-of-life planning, and adjustment to hospice care.
• Help patients and families process fear, anxiety, depression, denial, guilt, conflict, and other emotional distress associated with disease progression and end-of-life care.
• Provide crisis intervention when necessary and promptly escalate urgent concerns to the appropriate clinical leader.

Care Coordination

• Collaborate with the RN Case Manager, Chaplain, Hospice Aide, Medical Director, Administrator, facility staff, and other members of the hospice team.
• Participate in care planning and help ensure that identified psychosocial needs are incorporated into the patient’s Plan of Care.
• Coordinate with hospitals, nursing facilities, assisted living facilities, families, community organizations, and other agencies as needed.

Community Resources and Family Support

• Connect patients and families with appropriate community resources and support services.
• Assist with long-term care planning, advance directives, Medicaid and community assistance, funeral-planning resources, caregiver support, and other psychosocial needs.
• Educate patients and families about hospice benefits, levels of care, patient rights, caregiver responsibilities, and available support services.
• Help patients and families understand the hospice philosophy and the purpose of comfort-focused care.

Advance Care Planning

• Support conversations concerning advance directives, medical power of attorney, DNR status, end-of-life wishes, funeral arrangements, and family decision-making.
• Notify the clinical team of any conflict, confusion, or change in the patient’s or family’s goals of care.

Documentation and Compliance

• Complete all documentation accurately, professionally, and on time in the Kinnser/WellSky electronic medical record system.
• Ensure that social work notes clearly describe the patient’s psychosocial condition, caregiver burden, emotional needs, interventions provided, family response, and follow-up plan.
• Document thoroughly to support hospice compliance, continuity of care, continued hospice eligibility, and Additional Documentation Request (ADR) readiness.
• Submit visit notes and all other required documentation within agency-established timeframes.
• Maintain patient confidentiality and comply with HIPAA, agency policies, and applicable professional standards.

Interdisciplinary Group Participation

• Attend and actively participate in Interdisciplinary Group meetings as required.
• Provide meaningful psychosocial updates for assigned patients.
• Report changes in patient or family coping, caregiver availability, emotional distress, safety concerns, financial or resource needs, and unresolved psychosocial issues.
• Recommend appropriate updates to the Plan of Care based on the changing needs of patients and families.

Required Qualifications

• Current Texas social work license in good standing; Licensed Master Social Worker (LMSW) is preferred.
• Minimum of one year of social work experience in a healthcare setting.
• Knowledge of hospice philosophy, end-of-life care, grief, family dynamics, caregiver stress, and psychosocial assessment.
• Strong communication, counseling, problem-solving, and crisis-intervention skills.
• Ability to complete accurate, timely, and professional clinical documentation.
• Ability to work independently while collaborating effectively with an interdisciplinary hospice team.
• Ability to maintain appropriate professional boundaries.
• Demonstrated compassion, patience, professionalism, and cultural sensitivity.
• Ability to successfully complete all required background checks, license verification, and onboarding requirements.
• Commitment to maintaining confidentiality and complying with HIPAA and all agency policies.

Work Expectations

• Complete assigned visits in accordance with each patient’s Plan of Care.
• Submit documentation within Millenia Hospice’s required timeframes.
• Attend IDG meetings and provide meaningful updates regarding assigned patients.
• Promptly communicate changes in psychosocial condition, caregiver concerns, safety risks, family conflict, or other significant issues.
• Follow all Millenia Hospice policies, procedures, HIPAA requirements, and professional standards.
• Represent Millenia Hospice with compassion, professionalism, dignity, and respect at all times.
