What You'll Do:
- Screen, evaluate, and treat patients with communication, cognitive, or swallowing disorders
- Develop and implement individualized care plans following clinical standards
- Utilize standardized assessments and evidence-based practices
- Monitor patient progress and modify treatments accordingly
- Communicate findings and progress with the healthcare team
- Complete thorough discharge summaries and maintain accurate documentation
- Participate in care conferences, training, and team meetings
- Support clinical supervision of Cfys or students as needed
What You Need:
Required:
- Valid state licensure as a Speech-Language Pathologist
- Active ASHA membership and licensure compliance
- 1+ year experience in LTC preferred
- Excellent communication and clinical skills
Preferred:
- Prior experience with diverse patient populations
- Ability to collaborate effectively with interdisciplinary teams
