Home Health Physical Therapist, PRN Halcyon Home LLC, Spring, Texas, United States Position Overview Halcyon Home seeks an experienced Home Health Physical Therapist to provide care to our geriatric clients in Tomball/Spring, TX and surrounding area. The role involves ongoing clinical assessment, developing and evaluating plans of care, documenting treatment, and providing patient and caregiver education. Key Responsibilities Perform ongoing clinical assessments, including OASIS assessments, to identify patient needs and determine level of functioning. Develop and evaluate plan of care in partnership with patient, representatives, and caregivers. Document clinical notes, phone orders, and other records on an ongoing basis and in a timely manner. Provide services ordered by the physician as indicated in the plan of care and oversee other team members when applicable. Apply infection control and standard precautions in patient care activities. Accept clinical assignments consistent with education and competence to care for patients. Provide supervision and guidance to physical therapy assistants and home health aides. Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment. Communicate with all physicians and other health care practitioners involved in the plan of care. Meet mandatory continuing education requirements of the Agency and licensing board. Assist and instruct patients and families on transfer techniques and use of equipment such as walkers, wheelchairs, and canes. Provide patient, caregiver, and family counseling and education. Salary Information $75 - $150 hourly wage Requirements Current PT license Knowledge or experience utilizing Homecare Homebase EMR preferred Experience in home health or related healthcare setting Strong clinical and assessment skills Excellent communication and interpersonal abilities Ability to work independently and manage time effectively.
