Position Summary Primary City/State: Wheatland, Wyoming The Case Manager in Care Coordination will provide comprehensive care coordination for patients as assigned. The role is accountable for the clinical quality of Care Coordination services delivered by both the case manager and others, and identifies/resolves barriers that may hinder effective patient care. Responsibilities include conducting initial assessments, participating in daily multi-disciplinary rounds, communicating discharge plans and updates to patients, families, and the medical team, and delegating tasks to a transitional care associate. This position is PRN, worked 2 days per week, 16 hours per week (8:00am – 5:00pm). Core Functions Manages individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes. Acts in a leadership role with process improvement activities for populations of patients to achieve optimal outcomes. Collaboratively develops and manages interdisciplinary patient discharge plans, communicating the plan across the continuum of care. Maintains knowledge of Medicare, Medicaid, and other program benefits to assist patients with discharge planning and choices; understands community resources relevant to health care, end‑of‑life dynamics, substance abuse, neglect, and domestic violence. Establishes and promotes collaborative relationships with physicians, payers, and other members of the health care team; collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements. Educates internal members of the health care team on case management and managed care concepts, facilitating integration into daily practice. May supervise other staff. Has freedom to determine how to best accomplish functions within established procedures, consulting supervisor on any unusual situations. Minimum Qualifications Registered Nurse (RN) : Must possess a bachelor’s degree in case management or health care and hold a current RN license in the state of employment. Requires a proficiency level typically achieved with 2 years of clinical experience and demonstrated critical thinking, problem‑solving, communication, and time‑management skills. Must be able to work effectively in an interdisciplinary team format; may be required to take rotating call based on acute facility need. For acute care assignment, a minimum of one year case manager experience in an acute care hospital is required. Social Worker : Requires a Master’s Degree in Social Work and a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license (or equivalent). If not licensed at hire, must obtain within 6 months of hire date. For acute care assignments, Basic Life Support (BLS) certification is required. Preferred Qualifications Certification for Certified Case Manager (CCM) preferred. Additional related education and/or experience preferred.
