Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996. Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas. Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas POSITION SUMMARY To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers. ESSENTIAL FUNCTIONS Assess the psychosocial needs of the patient and family members Provide therapeutic counseling and emotional support to patients and family members, and education on end of life issues Contribute to and execute the Plan of Care with the family/patient Record daily documentation of visits and contact in accordance with Policy Provides referrals to community resources Perform other duties as assigned POSITION QUALIFICATIONS Bachelor's of Social Work with 1 year of applicable experience required; Master's degree preferred Reliable transportation and a valid driver's license. Hospice or healthcare experience preferred.
