Responsibilities
- Perform initial and ongoing patient assessment and develop an individualized plan of care in collaboration with patients and families; monitor progress and adjust plans as needed.
- Coordinate patient care with the multidisciplinary team, supporting discharge planning and transitions of care to optimize outcomes.
- Document nursing assessments, plan of care, interventions, and patient responses in the electronic medical record (EPIC) with accuracy and timeliness.
- Educate patients and families about the care plan, treatment options, and self-management, providing written materials as appropriate.
- Identify barriers to care, communicate issues to the care team, and coordinate resources to address clinical and social needs.
- Ensure compliance with applicable policies, standards, and legal requirements while maintaining safe, high-quality nursing care.

Requirements
- Active Registered Nurse license in California (or state of practice)
- Minimum two years of recent RN experience in case management or a related field
- Current CPR/BLS certification
- EPIC EMR experience required
- Valid driver's license; must be at least 21 years old; current automobile insurance
- Bachelor of Science in Nursing (BSN) preferred
- Ability to function effectively in a hospital setting and collaborate with an interdisciplinary team
