RN Case Manager: Discharge & Care Coordination

Saint Francis Health System is seeking a Case Manager (RN) to provide case management services, identify discharge needs, and coordinate discharge planning with the care team. The role focuses on safe transitions of care and reducing readmission risks. The RN must have a nursing degree/diploma and an Oklahoma license (CCM preferred).
