Job Overview - Care Coordinator
Compensation: $26/hour
Location: New York, NY
Schedule: Monday to Friday (In-Office)


TAG MedStaffing is hiring a Care Coordinator in New York, NY for our client, supporting care management, service coordination, member advocacy, and access to healthcare and community resources. The Care Coordinator will work directly with individuals to implement care plans, coordinate essential services, and monitor ongoing needs through regular outreach and field-based visits. This role requires experience supporting homeless and HIV-affected populations within a care management or human services environment.


Responsibilities as the Care Coordinator:

• Care Coordination: Coordinate healthcare, behavioral health, housing, social services, and public benefit resources based on individualized care plans.
• Member Advocacy: Advocate for members and help address barriers affecting access to appropriate healthcare and community-based services.
• Field Support: Conduct home and community visits to monitor member needs, service engagement, and progress toward established care goals.
• Service Navigation: Connect members with appropriate providers and resources while maintaining productive relationships with community service organizations.
• Case Collaboration: Participate in case conferences, supervision, and training while collaborating with multidisciplinary teams on member needs.
• Documentation: Maintain accurate and timely case records, service documentation, member charts, and billing-related activity logs.

Qualifications for the Care Coordinator:

• Education: High school diploma or GED is required.
• Licensure: Valid driver's license with a clean driving record is required.
• Experience: 2+ years of care management, case management, or related human services experience is required.
• Industry Knowledge: Experience supporting homeless and HIV-affected populations is required, along with knowledge of healthcare, behavioral health, housing, and community-based resources.
• Technical Skills: Proficiency with electronic documentation, data entry, and standard office technology is required.
• Skills & Attributes: Strong written and verbal communication, advocacy, organization, interpersonal, and independent case management skills are required, with bilingual Spanish skills preferred.
