Our culture is the foundation for a patient-centered medical home where everyone in our community can get easy access to superior care.DETAILS:- Assesses patient status by obtaining health history through patient/family interviews and chart reviews- Orders/performs appropriate laboratory diagnostic and other screening tests- Develops and implements a treatment plan by prescribing/dispensing medications and/or injections - Two Full-Time Highly Trained Clinical Support Staff plus case managers to assist you in your practice- Bilingual (SPAN-ENG) candidates encouraged to apply- Our patient population is primarily medicare advantage- EMR: Athena
