Physical Therapist – Per Diem

Position Summary

Avir at the French Home is seeking a compassionate and skilled Physical Therapist (PT) to join our team on a per diem basis. The Physical Therapist will provide evaluation and treatment services designed to help residents improve mobility, strength, balance, functional independence, and overall quality of life.

Essential Responsibilities

• Evaluate residents to identify physical limitations, mobility concerns, and functional needs.

• Develop individualized treatment plans based on each resident's condition and goals.

• Provide therapeutic exercises, mobility training, balance activities, gait training, and other appropriate interventions.

• Monitor resident progress and modify treatment plans as needed.

• Educate residents, families, and caregivers on exercises, mobility techniques, and safety.

• Collaborate with nursing staff, physicians, occupational therapists, speech therapists, and other members of the care team.

• Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements.

• Promote resident safety and independence during all therapy activities.

• Participate in care planning and communicate changes in resident status to the appropriate team members.

• Follow professional, ethical, and facility standards of practice.

Qualifications

• Current Physical Therapist license in the State of Texas .

• Graduate of an accredited Physical Therapy program.

• Current CPR certification, as required.

• Strong clinical, communication, organizational, and interpersonal skills.

• Experience working with older adults or in a skilled nursing/rehabilitation setting is preferred.

• Ability to work independently while collaborating effectively with an interdisciplinary team.
