Hospice Chaplain

Gentiva is seeking a compassionate Hospice Chaplain to provide spiritual and emotional support to patients and families in their homes. In this PRN/as-needed role, you will perform spiritual assessments, offer counseling, prayer, and grief support, and collaborate with the hospice interdisciplinary team to honor patients’ beliefs and preferences. You will document visits, participate in care planning, and provide support during end-of-life and bereavement. This role is ideal for a mission-driven chaplain who values dignity, empathy, and teamwork in a home-based hospice setting.
